Qualifying Paper (QP) Forms

• QP Information Form - by the end of the 3rd/5th semester
• QP Defense Announcement- by three weeks before the defense (QP2 only)
• QP Defense Report - on the defense date (by the defense chair)
• QP Signature Page - after the final version is accepted by the committee
• QP Completion Report - after all committee signatures are collected
• QP Extension Request- when necessary (consult GPD before submitting this form)
