External Technical Elective Request

Plesase submit this form if you want to request a course external to BME to count as a technical elective for your M.S. degree.

 

BME Qualifying Exam Letter of Intent

Plesase submit this form if you plan on taking the qualifying exam, and only after attending the QE orientation.

 

Ph.D. Proposal, Ph.D. Final Defense and M.S. Final Defense Committee Request

Use this form to request approval for the committee for your Ph.D. proposal defense and your Ph.D. final defense, or your M.S. final defense.  We will confirm the approval of your committee.  For your Ph.D. proposal defense or M.S. final defense, there is no submission deadline. For your Ph.D. final defense, you must submit this form at least 4 weeks prior to the defense date.

 

Doctoral Dissertation Abstract Form

Use this form to submit your doctoral abstract for your final defense.  You must submit this form at least 4 weeks prior to the defense date. The abstract is a lay abstract with 350 words or less.

 

Announce Your Defense to the program

Use this form to submit information about your Ph.D. Proposal Defense, Ph.D. Dissertation Defernse or your M.S. Thesis Defense.  We will forward this information to all students and faculty, so please ensure the accuracy of the information.  Also, please submit this to us 2-3 days prior to your defense.

 

Good Standing Letter Request

If you require a good standing letter from the graduate program, please submit this form.  We will review the information and forward you the approved letter. 

 

BME Letter of Completion

If you need a letter of completion from the BME program, please complete this form. This is only for M.S. students and Ph.D. students who DID NOT submit the final dissertation to the graduate school. For Ph.D. students who have submitted their final dissertation to the graduate school, please request a letter of completion from the graduate school.
