Deadline Approaching - Submit Your Full Proof of COVID Vaccination to Remain in Your In-Person Fall Classes
Dear Seawolves,
Our records indicate that you havesubmitted only partial proof (first dose) of your COVID vaccination. To be in compliance with the SUNY requirement to provide proof of full vaccination and remain enrolled in your fall in-person classes and live on-campus (if you are a residential student), you must submit proof of your full COVID vaccination to the Wolfie Health Portal by the dates noted below depending upon when you received your first dose:

If you received your first dose of Pfizer at least 21 days ago, or your first dose of Moderna at least 28 days ago, you must complete your second dose if you have not already and submit your full proof of vaccination by October 2 to the Wolfie Health Portal or you will be deregistered from your fall in-person classes and restricted from living on-campus (if you are a residential student).

If you received your first dose of Pfizer within the last 21 days, or your first dose of Moderna within the last 28 days, you must complete your second/final dose and submit proof of vaccination to the Wolfie Health Portal no later than 3 days after you are eligible. For example, if you received your first Pfizer dose on September 15, you are eligible to receive your second Pfizer dose 21 days later - on October 6, and must obtain that dose and submit proof of vaccination to the Wolfie Health Portal by October 9, or you will be deregistered from your in-person classes and restricted from living on-campus (if you are a residential student).

NOTE: If you believe you have already submitted proof of your full COVID vaccination to
the Wolfie Health Portal, please complete this FORM by September 29 and our team will check your records and reply to your Stony Brook
email address within 1 business day.

COVID vaccines are widely available at pharmacies, local health departments, clinics, and other locations across the state. Visit Vaccines.gov to find appointments near you or contact your local pharmacy or provider.
